Home Care Package consumers and NDIS participants can choose to self-manage their funding packages and thus have more control over their support services and how they are provided. Self-management does require a good understanding of package management tasks and attention to detail.

It is essential that older people, people with disability and their carers and families who are considering or doing self-management understand the requirements, processes, benefits and the pitfalls.

This half day workshop covers:

  • What is self-management within Home Care Packages and the NDIS and the benefits for consumers and participants
  • what people need to think about and the systems they need to have in place if they intend to self-manage
  • negotiating with service providers on support provision
  • financial and other risks in package self-management

This session will be followed by a Showcase where attendees will have the chance to meet and discuss with service providers.

1.30 pm to 4.30 pm: Getting to Client-Set Goals: Engaging and Motivating Clients

Target Audience: This session is designed for Case managers, Community Care Assessors and Service Coordinators

This workshop explores how information gathered in referral, intake and assessment processes is then used in combination with motivational interviewing (MI) techniques as the basis for co-designing client-set goals and person centred plans. These techniques are particularly relevant for consumer directed, person centred and wellness and reablement approaches.

The workshop will cover:

  • engaging consumers in their aged care journey
  • identifying support goals and co-designing support plans
  • strategies for building and maintaining motivation of consumers

About your presenter:

Glen Sorensen is the Managing Director of Age Communications, the company he founded in 1988. He has worked for over 35 years in the disability and aged care sector with all levels of government, home care and residential care providers.

Glen is a specialist in community care provision and in applying consumer-directed approaches so that consumers get the best support within their available budgets. He regularly conducts workshops and in-services for managers, assessors, support planners, support workers and volunteers and is increasingly running information sessions for consumers on navigating the service system and making it work for them. He is the author of two books, When I'm 64 and Options for the Ageing: The Complete Guide To The Support And Care Of Older People.
